Two 13-year-old boys crashed an allegedly stolen car into a fence during a police chase in Lithgow on Thursday morning.
The teenagers have been taken to Sydney's Westmead Hospital in stable conditions.
NSW Police has launched a critical incident investigation, which will be overseen by the Law Enforcement Conduct Commission.
Police say two 13-year-old boys who allegedly crashed and rolled a stolen car during a pursuit in the state's Central Tablelands were "extremely lucky" to have survived.
The pursuit followed reports that two males had broken into a home on Union Street in Lithgow in the early hours of Thursday morning and stolen the occupant's car.
Western Region Commander Andrew Holland said one of the teens allegedly brandished a large knife at the home owner.
"It's very distressing for [the home owners], waking up in the middle of the night to two offenders breaking into their home."
Officers began the pursuit after they tried unsuccessfully to stop the vehicle on the Great Western Highway.
Commander Holland said the car allegedly reached speeds up to 50 to 60 kilometres per hour above the speed limit during the short pursuit before it crashed into the fence of a home that backs onto the highway, near Lockyer Street at Bowenfels.
Commander Holland said emergency services cut off the roof of the car to extract the teenagers.
"They both have very serious injuries to their lower legs, broken bones and lacerations."
The 13-year-olds were treated by paramedics at the scene before being taken to Westmead Hospital and Sydney Children's Hospital in stable conditions.
The incident closed the Great Western Highway for several hours from about 2am.
Police are yet to interview the teens.
Commander Holland said youth crime was a big concern for the police.
A critical incident team from the Central West Police District will lead an investigation into the incident.
It will be reviewed by the Professional Standards Command and overseen by the Law Enforcement Conduct Commission.
